Guidelines regarding various services for Citizens in SUWIDHA Centre

Monday to Friday from 09:15am to 05:00 pm. Lunch Break 01:30 pm to 02:15 pm. | |

Forms: |
Form for particular service are available on counter No. 23. Purchased by the applicant and application to be filled by the applicant in clean hand writing. Documents required with the application as per the yellow slip (Check Sheet) be attached. |

Facility to fill the forms: |
If you are finding it difficult to fill the forms, you can approach the counter No. 21 to fill the form. There are no fees to fill the form. |

Photo copy and Stamp Paper: |
Photo copy, stamp papers, Tickets for the Stamp papers and Court fees Stamps are available on counter No. 22. |

Documents required for Identity proofs: |
Affidavits required to be attached with the application and to get them attested from the Executive Magistrate, you should produce any one document i.e. voter identity card, Driving license, Arm license, Government/Semi Government identity card, Ration Card with photograph/family photograph, Passport, as identity proof of the applicant or identified by the MC of the ward/Village Nambardar/Sarpanch/Govt Officer/Employee who has the identity card with him and his photograph along with the applicant will be taken at the counter. |

Allotment of Q-No.: |
The applicant, after filling the form, should approach at counter No. 19&20 and obtain Q and Counter Token No. |

Submission of Application: |
Applicant should report at the allotted counter No. and watch for his Q-No displayed on the Q-Display Unit. In at the Counter case the Q-No is far of he/she should sit down on the chair provided in the hall and wait. On display of your No., you should approach to the counter operator and submit the application and pay the required fees. The counter operator will give you the computerized receipt which you should collect from him. |

Delivery of Documents: |
The applicant should approach for delivery of document on the date mentioned on the computerized receipt given at the time of submission of application at the delivery counter (Counter No.3 for I-cards of Government Employees, Sarpanch/Numbardar/Panch/Panchayat Secretary and Mc), Driving license(New, Learning, Renew, Duplicate, Endorsement licenses), Registration of vehicle (New Transfer, Duplicate, H.P Endorsement, H.P Cancellation), Small Saving license, Cast Certificate (S.C, B.C, ), Residence Certificate. Copying (Copying of Marriage Certificate, Copying Notation, Copying Musafi. H.R.C (Register Deed, Unmarried Certificate), R.TI related documents. The applicant should personally collect his/her own document by producing the original computerized receipt. In case he/she can not come to collect the document personally, and then he/she should authorize the person along with his/her signatures on the backside of original computerized receipt. In case the original computerized receipt has been lost/misplaced he/she should write an application address to the Project Manager with the request to deliver the document along with his/her identity proof (One only) as mentioned above. |

Information by Telephone: |
The applicant must give his/her Landline Telephone/Mobile No. to the Data Entry Operator. The status of application will be conveyed to the applicant on his/ her Mobile No. through S.M.S service. In case the applicant wants to know his/her application status he/she should send S.M.S on No. 9417607833 In case he/she does not have Mobile Telephone he/she should ring up the helpline at Telephone No. 01822-230666. |

Note:

Avoid contacting Agents, Middlemen and Touts they may misguide you as no application without proper documents as per requirements will be accepted in SUWIDHA Centre.

SUWIDHA Centre is for you and please make sure that you do not litter any paper and eatables on the flours, also to ensure that bathrooms provided on the side of the SUWIDHA Centre (meant for public convenience are only used).

In case you have any complaint/suggestion there is a suggestion box the applicant can drop the same in that or he/she can get it entered in the complaint register kept at the Reception Counter.

The applicant regarding non-delivering documents by due date can get his/her complaint endorsed in the register kept at the Reception counter so that the reminder to the concerned Department/Branch.

The applicant should approach the Reception Counter at SUWIDHA Centre for all queries. In case he/she still not satisfied he/she can approach the Project Manager SUWIDHA Centre personally or on Telephone No. 01822- 236663.

In case the applicant finds abnormal delay in delivery of his or her document he/she can approach the Additional Deputy Commissioner (G)/Deputy Commissioner personally.
